Lower-protein diet linked to toxin control in incremental PD

In an interim analysis of 205 patients starting incremental peritoneal dialysis—the lower-dose approach examined by the I-COPE PD study—a prescription of ≤0.6 g/kg/day of protein was associated with lower serum urea, phosphate and potassium over one year.

Silvio Borrelli, M.D., Ph.D., of the University of Campania “Luigi Vanvitelli” in Naples, Italy, and colleagues divided the patients into two groups: those prescribed ≤0.6 g/kg/day and those prescribed more than 0.6 g/kg/day. The lower-protein group represented 45.4% of the cohort. The researchers compared measurements at baseline, six months and 12 months.

The signal was specific. Serum urea, phosphate and potassium were significantly lower in the lower-protein group despite comparable dialysis adequacy. Serum albumin followed a similar course in both groups, with an initial decline followed by stabilization, and BMI showed no significant difference. Persistence with incremental dialysis was also similar: 59.1% versus 63.3%.

And so what, concretely? For patients starting peritoneal dialysis at a lower dose, the findings suggest that a lower protein prescription could help control several uremic toxins without an obvious difference in albumin trajectory, BMI or the likelihood of remaining on the incremental approach. That could give clinicians another dietary option to study alongside dialysis planning.

The limits matter. This was an interim, observational cohort analysis, so it shows an association rather than a cause-and-effect result. The study was partially funded by Vantive, and two authors disclosed ties to biopharmaceutical companies. The authors describe the findings as suggestive evidence of benefit and safety, not as a clinical verdict.
