Pancreatic cysts show sharply different three-year cancer risks
A scan can uncover a pancreatic cyst while doctors are looking for something else. Most never become cancerous, but some carry imaging features that demand a harder decision: operate now, or watch and wait. Mayo Clinic researchers have now estimated the three-year risk of pancreatic cancer or advanced precancerous changes across those warning levels.
The prospective study followed 230 people with pancreatic cysts classified as having worrisome or high-risk features. The risk was about 2% for patients with one worrisome feature, 4% for those with two and 18% for those with three or more. The pattern suggests that several warning signs identify a substantially higher-risk subgroup, while one alone was not markedly associated with elevated risk.
The study design matters because participants were enrolled and followed over time whether they underwent surgery or entered surveillance, rather than being drawn mainly from past medical records or surgical cases. Of the 230 participants, 32 underwent surgery within the first three months, while 198 entered clinical surveillance. Cancers diagnosed during careful surveillance were found at a very early stage, suggesting that appropriately selected patients may have an alternative to immediate surgery.
And what does that change in practice? It gives doctors and patients a clearer number to discuss when weighing surveillance against an operation. For someone with one worrisome feature, the estimated risk is far lower than for someone with three or more; that distinction could help reduce unnecessary procedures while directing closer attention to patients at greater risk. The study does not remove uncertainty, and the estimates still need to inform larger studies.
Blood tests are not yet a shortcut. In a separate study, an elevated CA 19-9 result — a blood biomarker sometimes used when monitoring pancreatic cysts — did not reliably predict cancer on its own; among patients with low-risk cysts and an elevated result, none developed pancreatic cancer during follow-up. Mayo Clinic researchers say the data can help design trials of new biomarkers and more personalized early-detection strategies.
