Story tests uncover ‘chemobrain’ missed by standard tests

A list of word pairs — “shoe-banana” and “book-fingers” — looked ordinary on a computer screen. But when female breast cancer survivors were asked to remember details from short everyday stories, they recalled fewer than women of the same age without a cancer history. The finding, from researchers including University of Ottawa associate professor Melanie Sekeres, suggests that a routine memory test can miss the difficulties survivors notice in daily life.

The study examined women at least six months after chemotherapy treatment, pairing them with age-matched controls for two online testing sessions. On the standard verbal paired-associate test, survivors performed similarly to the control group. On the Taler Stories test, which asks people to recall details from events described in story form, the gap appeared.

That difference matters because the reported problem is not limited to memorizing isolated words. Survivors have described trouble remembering personal events, following conversations, switching between tasks and responding quickly. The story-based test places memory inside an everyday sequence, making it closer to the kind of recall that can demand extra mental effort outside a clinic.

The result is evidence of a measurement gap, not proof that every survivor will experience the same impairment. The research focused specifically on women treated for breast cancer and used online testing. Its authors say standard memory assessments remain essential; their proposal is to supplement them with tools that better reflect real-life demands.

So what changes in practice? If these findings hold up, survivor care could pair conventional cognitive screening with story-based tasks, giving clinicians a better chance of detecting “brain fog” that a word-pair score overlooks. That could make it easier to identify long-term challenges and consider targeted support, including dietary support, physical activity and stress reduction.
