Aerobic exercise improves executive function after menopause

A brisk walk, a cycle ride or a water-aerobics class can now be measured against a specific cognitive target: the ability to switch tasks and manage mistakes. In a Rutgers-led randomized trial, 93 healthy women aged 20 to 67 spent six months in either an aerobic exercise program or a stretching-and-toning program, and postmenopausal women in the aerobic group made the strongest gains in executive function.

Executive functions are the skills that help people plan, stay focused, organize activities and move from one task to another. The study, led by Sharon Sanz Simon of Rutgers New Jersey Medical School and published in the Journal of Alzheimer's Disease, found measurable improvements after three months, with benefits continuing to strengthen through the six-month trial. The gains did not appear in memory or processing speed.

The participants were not already active: all began with below-average cardiorespiratory fitness and did not exercise regularly. That makes the result relevant to the women the researchers wanted to reach, rather than only to people already following a demanding fitness routine. About 8 in 10 completed at least three months, and about 7 in 10 completed the full six months.

The practical program outlined by Rutgers calls for three to four one-hour sessions per week, maintained for at least three to six months. Each session includes a 10- to 15-minute warm-up and gentle stretching, followed by 45 minutes of continuous moderate aerobic activity. Brisk walking, light jogging, cycling, elliptical training, swimming, water aerobics and dance-based cardio are all listed as options; consistency matters more than the specific activity, with intensity increased gradually.

And so, concretely: postmenopausal women starting from a low fitness baseline may have a structured way to train cognitive flexibility alongside cardiovascular fitness, without needing one particular sport. The result does not show improvements in every thinking skill, and it comes from a single six-month study; Rutgers researchers say further work is needed to understand how lifestyle interventions preserve cognitive health across the lifespan. The researchers frame the issue as a significant midlife health window: two-thirds of Alzheimer's disease diagnoses occur in women, while menopause-related changes can coincide with more difficult thinking skills.
