Early statins after diabetes diagnosis linked to lower dementia risk

For people with type 2 diabetes, researchers examined whether the timing of statin treatment was associated with later dementia. In Danish health registers, researchers followed 132,585 statin-naive people who developed type 2 diabetes between 2006 and 2019, examining whether early statin initiation was associated with a later diagnosis of dementia.

The result appeared at ESC Congress 2026 and in The Lancet Regional Health – Europe. During a median follow-up of 7.1 years, 2.7% of participants developed dementia. Compared with people who did not start statins within five years of their diabetes diagnosis, those who began treatment within one year had a 15% lower relative risk of dementia over 10 years. Starting between one and five years after diagnosis was associated with a 10% lower relative risk.

Researchers divided patients by when treatment began and used a clone-censor-weight design. That method was chosen to emulate a randomized trial and reduce some of the bias common in observational research. The association was similar in women and men, but the study did not test statins in a randomized experiment.

The practical change is modest but concrete: for people with type 2 diabetes and high LDL cholesterol — the form of cholesterol the treatment lowers — early statin use is already relevant to cardiovascular risk and may also be linked to a lower dementia risk. Statins are widely available and inexpensive, yet the researchers say they appear to be underused in this group. The finding can therefore add one more point to discussions between patients and clinicians about when to begin treatment.

The boundary matters. The figures come from health records, not a trial assigning patients to statins or no statins, so they cannot prove that the drugs prevented dementia. Professor Isabel Goncalves of the European Society of Cardiology described the reduction as modest and said the results should support further research rather than settle the question.
