AI finds seven-protein signature of vasculitis remission

More than 100 patients at Charité in Berlin gave researchers blood-plasma samples for a difficult clinical question: had their vasculitis gone into remission, or was the disease still active? About half were in remission, alongside a group of healthy controls. The team has now identified a combination of seven proteins that reliably indicated remission, and confirmed the finding in a separate group of around 100 patients in Prague.

Vasculitis occurs when the immune system attacks blood vessel walls. The resulting inflammation can severely damage organs, including the kidneys and lungs. Immunosuppressive medicines can control the disease, but they may also cause serious side effects, making the timing of treatment reduction consequential for patients. Existing markers such as CRP, an inflammatory blood marker, cannot reliably establish remission.

The Berlin collaboration combined proteomics—the large-scale measurement of proteins in blood—with machine learning, a form of artificial intelligence. Researchers first found several hundred proteins whose levels varied with disease status. They then removed signals that might confuse the model, such as proteins that reflected inflammation without specifically indicating remission, before selecting the seven-protein combination.

The result is a research signature, not yet a test available in ordinary clinics. The researchers say the causal relationships between the seven proteins and vasculitis are still unclear. They also need larger studies to determine how the assay performs in routine clinical practice and whether it improves outcomes for patients.

And so, concretely? A reliable remission signal could help physicians decide when immunosuppressive treatment may be reduced or avoided, potentially limiting exposure to side effects. For now, patients and doctors still lack a widely available test, and the signature remains at the study stage. The same proteomics-and-AI strategy could, in principle, be adapted to other immune and inflammatory diseases.
