Community blood-pressure care cuts dementia risk by 15%

The trial took place across 326 rural Chinese villages, where community health care providers managed blood pressure with physician supervision. After seven years, the intensive-care group had a 15% lower risk of dementia than people receiving usual care, according to results from the China Rural Hypertension Control Program presented at ESC Congress 2026.

The trial enrolled 33,995 adults aged 40 or older with high blood pressure. Providers initiated and adjusted medicines through a simple stepped-care protocol, targeting SBP below 130 mmHg and DBP below 80 mmHg.

The difference in blood pressure was substantial. At the end of the seven-year study, SBP had fallen by 17.6 mmHg in the intervention group, compared with 2.4 mmHg with usual care. Dementia incidence was 8.85% with intensive treatment and 10.55% in the comparison group; the adjusted risk ratio was 0.85, with a 95% confidence interval from 0.78 to 0.91.

The results extended beyond dementia. Intensive blood-pressure control was associated with a 13% lower risk of cognitive impairment without dementia, and serious adverse events occurred in 47.15% of the intervention group versus 49.78% of those receiving usual care. The program had four active years, followed by three years of observation only.

Concretely, the finding gives primary-care systems a way to act on a modifiable dementia risk factor without relying exclusively on physicians for every medication adjustment. That matters because the intervention used trained community providers and a standardized protocol. Its reach is promising, but the evidence remains tied to rural China and this trial population; it does not by itself establish the same outcome in every health system.
