Researchers propose 10–20-minute blood test for bile acids

A vial of blood could carry an early warning that routine checkups currently miss. Researchers at the University of Toledo report a test that identifies elevated bile acids in 10–20 minutes, using just four additional microliters of blood collected alongside a glucose test.

Bile acids help the body digest fats and absorb fat-soluble vitamins. Normally, they circulate between the liver and intestine rather than building up in the bloodstream. When the liver is impaired or bile flow is blocked, they can accumulate in blood—a condition known as cholemia—before fatigue, abdominal discomfort, jaundice or familiar blood markers appear.

The discovery came from an unexpected change in red blood cells. The researchers had expected exposure to bile acids to make the cells more fragile. Instead, in several mouse models and in human samples, the cells became more resistant to rupturing. The team says bile acids cause the cell membrane to accumulate more cholesterol while losing its major phospholipid, making the membrane more rigid and detectable through an osmotic-stress test.

The human comparison involved 23 patients with cholestatic liver disease treated at the University of Toledo Medical Center and 23 age- and gender-matched controls without signs of liver disease. The test would not diagnose liver disease: a positive result would signal the need for additional evaluation. The university's Technology Transfer Office has filed a patent application for the diagnostic concept.

And so what, concretely? A first-line screen could give clinicians a reason to investigate liver dysfunction earlier, when monitoring or treatment may still have more room to help. The researchers point especially to intrahepatic cholestasis of pregnancy, where bile acids above 40 micromolar can cause severe fetal harm. The result remains a research finding rather than a routine clinical service, and the proposed screen would sit alongside—not replace—diagnostic workups.
